Subject Access Requests

Under the General Data Protection Regulation (GDPR) individuals have the right to obtain access to their personal data (and only theirs).

To request access to your records please complete the subject access request form below and return to the practice via email to manorsurgery@nhs.net

Please note that we aim to respond to requests within 28 days. If your request will take longer than this to process we will let you know within one month of receipt of the request.
